The former adult film actress was watching a Stanley Cup Playoff game earlier this year when the six-ounce disk crashed into her.
She said it ruptured her left breast implant and left her feeling like "blood was going everywhere".
The American-Lebanese presenter even kept the puck – that she believed could have been travelling 80mph – as a treasured souvenir.
Now the West Ham fan has revealed she will undergo surgery to fix her breast at the beginning of next year.
Mia Khalifa told Daily Star Online: "I was sitting behind the glass during a game, and it came shooting over the glass and it caught me so off guard and I had no idea it was coming.
"I grabbed my chest and I didn't want to let go, because I felt like if I did let go blood was going to be everywhere.
"I got to take it home, it was the single greatest souvenir any hockey fan can get: a game-used puck that comes at you and hits you.
"They're really heavy, it's pure rubber, they go at about 80mph.

"My left breast is slightly deflated now and I will be getting it fixed next year."
However Mia – who co-hosts Rooster's sports video podcast Sportsball – said it was not the most painful experience of her life.
Asked if it ranked as her most excruciating moment, she responded: "I've seen my team (Washington Capitals) lose before, that's a lot more painful."
Following the incident Mia – who played hockey and lacrosse growing up in Washington DC – wrote on Instagram: "I got hit in the boob with a puck during the game, and I’m 80% sure it ruptured an implant.
"But the good news is, I got a game used puck from a Capitals Stanley cup playoff game. Worth it."
Mia launched Sportsball earlier this month, targeting sports fans who "don't take themselves to seriously".
Earlier this week she launched a scathing attack on British cyclist Chris Froome, telling us she "f*****g hates" the Team Sky rider.

She also threw her support behind Loris Karius after his howlers in the Champions League final loss to Real Madrid, thought to be behind Liverpool's acquisition of new keeper Alisson who is expected to take his place.
But most surprising of all was Mia's admission that she is a huge West Ham fan earlier this month.
She told us: "I watched Green Street hooligans when I was young and ever since then I've loved West Ham.
"I haven't been to any games, that was actually my first time ever in England when I was there.
"I could see the stadium from where I was at the top of the tower, it looked a little far away.
"I would love to (attend a game), I actually plan on going back to do a food tour there.
"And I'm going to make it line up with a match that I can go and see."
She added: "They (fans in Green Street) were just so rough around the edges and a little badass, and also I love the song, Bubbles."
Mia's three-month spell as an adult film actress in 2014 shot her to fame, with her being ranked number one performer on adult site Pornhub.
She is still ranked number two four years later, and commands a whopping 8.9million followers on Instagram.
